Return authorization is required. Simply email Customer Service for a return authorization number and ship via insured mail back to Truekare. We gladly refund the purchase price (minus shipping charges).
Note on forms with adhesive backs: Due to the sanitary nature of these forms, we do not accept returns on any forms with adhesive backs.
